Cancer Cases Could Surge 67% Worldwide by 2050

The numbers are stark. In 2024, nearly 21 million people were diagnosed with cancer globally, and 9.8 million died from the disease. Now cases will reach 34 million by 2050 — a 67% increase driven almost entirely by population growth and aging, before any changes in risk factor exposure are factored in.

For anyone working at the intersection of oncology, public health, or life sciences, this report lands as both a warning and a challenge.

The Scale and Its Drivers

The findings are drawn from GLOBOCAN 2024 — the most comprehensive cancer surveillance database available — covering 34 cancer types across 186 countries and updated by IARC, the cancer agency of the WHO. The analysis was published in CA: A Cancer Journal for Clinicians, the ACS flagship journal.

Roughly 1 in 5 people alive today will develop cancer at some point in their lifetime. About 1 in 9 men and 1 in 13 women are expected to die from it. These are not projections shaped by worsening behavior or expanding risk — they are the arithmetic of a growing and aging global population. The underlying cancer risk is already baked in.

Where the Burden Falls

Cancer does not distribute itself evenly across geographies or populations, and this report makes those disparities unusually legible.

Incidence rates vary by a factor of four to five across regions. Australia and New Zealand report the highest rates; parts of sub-Saharan Africa and South-Central Asia the lowest — though lower incidence does not translate into better outcomes where diagnostic and treatment infrastructure is limited.

Mortality disparities are smaller in relative terms but still striking. Eastern European men and women in Melanesia face the highest death rates by region.

Lung cancer continues to dominate both incidence and mortality tables globally, accounting for 2.6 million new diagnoses and 1.9 million deaths in 2024. Tobacco remains its primary driver. Female breast cancer follows as the second most commonly diagnosed cancer — 2.4 million new cases, 694,000 deaths — and is now the leading cancer among women for both incidence and mortality worldwide.

One of the more instructive data points in the report concerns breast cancer survival. Women in Western Africa are twice as likely to die from the disease as women in Australia and New Zealand, despite having roughly half the incidence rate. The difference is not biological — it is structural. Access to screening, diagnosis, and treatment determines survival more than tumor biology in many settings.

Colorectal cancer ranked third in incidence and second in cancer deaths, with over 2 million new cases and 918,000 deaths. Liver cancer caused 732,000 deaths, the third-highest mortality figure globally. Among men, prostate cancer generated 1.5 million new cases, with death rates particularly elevated in the Caribbean and Sub-Saharan Africa.

Several cancers punch above their weight in mortality. Pancreatic cancer, ranked only eleventh for incidence, caused 491,000 deaths — placing it sixth globally for mortality. Late detection and limited treatment options continue to define its clinical reality.

Cervical cancer remains a problem the world knows how to solve and has not solved. HPV vaccination and screening exist. Yet cervical cancer is the leading cause of cancer death among women in 26 countries, most of them in sub-Saharan Africa, and across parts of South and Central America. The gap between available prevention and implemented prevention is the story there.

The Prevention Opportunity

This is where the report shifts register — from epidemiology to argument.

Dr. Hyuna Sung, lead author and senior principal scientist at ACS, frames the trajectory this way: “While the scale of the cancer burden and the stark geographic disparities are sobering, they also point to a tremendous opportunity. Nearly half of all cancer deaths are estimated to be avoidable through potentially modifiable risk factors, and an additional substantial proportion can be prevented through early detection and timely treatment.”

The list of modifiable factors is not new: tobacco use, cancer-linked infections, alcohol consumption, excess body weight, physical inactivity. Tobacco alone accounts for a disproportionate share of the projected burden. Dr. Ahmedin Jemal, co-author and senior vice president at ACS, calls for country-specific prevention plans while arguing that prevention must be every country’s top priority, regardless of income level or healthcare capacity.

The practical implication for the life sciences and healthcare ecosystem is significant. The coming decades will not merely demand more oncology care — they will demand smarter, earlier, and more equitably distributed care. Technologies that improve early detection, expand vaccination reach, reduce treatment costs, or identify preventable exposures at the population level will have outsized impact.

The 67% projection is not a forecast of what must happen. It is a description of what will happen without intervention. That distinction matters.
